Kinfolk Psychiatry Insurance and Billing Information

Before your first appointment, please verify your mental health benefits with your insurance provider. Confirm whether Joanna Sullivan, PMHNP-BC is covered as an in-network or out-of-network provider. Be sure to inquire about your annual deductible, if it has been met, the portion of the cost your insurance covers, your copay per session, and your maximum benefit for mental health services (dollar amount or number of visits allowed).

Deductibles & Payments
Insurance plans may have deductibles that reset each fiscal year. If you have a deductible, payment will be required at the time of service until it is met.

Co-insurance & Copays
If your plan includes co-insurance or a copay, these amounts will be billed at the time of service. Please ensure your insurance information is current to avoid denied claims. Any costs not covered by insurance will be your responsibility.

Out-of-Network & Self-Pay
If your insurance is not listed, we can still bill as an out-of-network provider or you can submit a Superbill for reimbursement. Any costs not covered by insurance are your responsibility. If you are uninsured or prefer not to use insurance, we are happy to bill you directly. Full payment is required at the time of service.

Self-Pay Rates

  • Initial intake: $450 - $550

  • Follow-up appointments: $250 - $350

Good Faith Estimate
If you do not have or are not using insurance, you are entitled to a Good Faith Estimate for the expected costs of non-emergency services, including related costs like medical tests and prescriptions. You will receive this estimate in writing at least one business day before your service. If the final bill is at least $400 more than the estimate, you have the right to dispute the charge.
